Please complete a 2009 Sunday School Registration Form for each child in your household. All information is important, so please be as accurate as possible.

Forms may be filled out and submitted online or downloaded as .pdf files and returned to the office. 4th & 5th grade registrations must include a signed medical release form to participate in field trips and outings. This can be downloaded below or picked up from the Children's Ministry Assistant..
